The AHCCCS web server automatically gathers certain information about the visit
and stores it in a standard log file.

are collected and stored in a log file and/or database. This information cannot

order to understand our visitor's needs.
The information is kept on a secure server within the AHCCCS office. This information

Some activities on our site require registration — for example, AHCCCS Providers
can create an account to verify member eligibility and enrollment. We ask you to
create an account where we collect additional personal information. We use this
information for internal security purposes only. This information is not shared,
sold, or otherwise disseminated by AHCCCS.

AHCCCS and its consultants have taken several steps to safeguard the integrity of
its computer systems and networks. These steps include, but are not limited to,

auditing, and encryption of information. Security methods and measures have been
integrated into the design, implementation and day-to-day practices of the entire
AHCCCS website.

or a survey with your personal information and submitting it to us from this website
